Form 10C
Meaning & Definition
Form 10C is an application filled out by employees to request benefits under the Employee Pension Scheme (EPS) if they leave work before satisfying their specified service time to begin receiving their pensions. The purposes of this application are to provide the employee with a withdrawal benefit or a scheme certificate (depending on the eligibility) as appropriate, and therefore ensure proper settlement of pension-related obligations.

Compliance Requirements
The Employee Pension Scheme, 1995, which is regulated by EPS and EPFO, provides for you to utilise Form 10C.
Form 10C can be used to provide a member with:
- If the member or employee leaves employment before qualifying for the monthly pension.
- If the member would like to receive a scheme certificate to present as evidence of eligibility for a future monthly pension.
